Transition care provides short-term care for older people to help them recover after a hospital stay. Care can last up to 12 weeks and take place in a person’s home, an aged care home or both.

Eligibility is determined by assessment abiding by the Aged Care Act (2024) and National Transition Care Guidelines. A current ACA approval is required for an older person to be admitted to TCP directly from hospital inpatient or the Hospital in the Home program.
TCP accepts referrals from all public and private hospitals. Please call TCP to discuss the referral process.
