Aged Care Assessment Team

The Single Assessment Service (SAS) – Aged Care Assessment Team (ACAT) helps older people access the aged care services and supports they need to remain safe, independent and well supported.

Our team works with older people, their families and carers to understand their care needs and determine eligibility for government-funded aged care services. Assessments are provided through the Australian Government's aged care system.

You may be eligible for an assessment if you:

• Are aged 65 years or older
• Need support to continue living independently at home
• Are considering residential aged care
• Require additional care following a hospital stay

People aged 50 years and over may also be eligible if they:

• Identify as Aboriginal and Torres Strait Islander, or
• Are homeless or at risk of homelessness.

An assessment can determine eligibility for a range of aged care services, including:

• Support at Home services
• Commonwealth Home Support Programme services
• Permanent residential aged care
• Residential respite care
• Transition Care Program
• Restorative Care Pathway
• End of Life Care Pathway
• Assistive technology
• Home modifications
• Other in-home care and support services.

The team provides assessments across hospitals in the Gold Coast and Beaudesert regions.

We assess medically stable patients who may require:

• Transition care after leaving hospital
• Residential aged care placement
• Additional aged care support services following discharge.

What happens during an assessment?

During the assessment, we will discuss:

• Your health and medical needs
• Daily activities and mobility
• Current supports and living arrangements
• Goals, preferences and care needs

We may also speak with your family member, carer or support person if you would like them involved. Assessments are designed to help identify the most appropriate services to support your independence and wellbeing.

A family member, carer, GP or healthcare professional can assist you with making a referral.
