Child and Youth Mental Health Service’s Continuing Care Team (CYMHS CCT) is a multidisciplinary team that includes psychiatrists, psychologists, social workers, occupational therapists, speech pathologists and clinical nurses who provide support to children and young people aged 0 to 18 years of age experiencing severe and complex mental health challenges. Our services include comprehensive mental health assessments, crisis management, individual therapy, family-focussed therapies and collaborative interagency care planning.
We prioritise recovery-focused, evidence-based and family-centred approaches and can offer additional supports from our carer peer support workers. Following initial contact, a child/young person and their family will be offered an assessment appointment where their presenting difficulties and history are explored and they will have an opportunity to identify concerns and care needs. Our team provides recommendations on the best suited treatment options and will engage with the young person and family to achieve those.

If you are a current CYMHS CCT consumer, or carer of a current consumer, you can reach out to your treating team via the contact details provided. We ensure that you and your family play a central role in all decisions and you can contact us to discuss a young person’s current care.
